Transfer Checklist

Has anyone yet implemented a transfer checklist in their PACU's for transfer to the inpatient unit? Who checks off the checklist? What parameters are included in your checklist?

We are attempting to design a checklist but are getting "blogged" down. Any assistance would be greatly appreciated. Thanks.

Here in North Bay, we have been using a written transfer ecord for about a year,The PACU nurses complete it prior to discharging the pt from PACU.We use it for all pts except myringotomies due to their short length of stay. Staff buy-in took several months as they felt it was repeated documentation

I was under the impression that the checklist is to be used to "track" verbal report, i.e. when giving verbal report, each section/item given is then "ticked" off in the box as heard and understood. I thought that only any notes the receiving nurse would like to keep for herself would be written. I didn't think the transferring RN wrote anything down. What do others think? What are your hospital policies?

At Trillium Health Centre in Mississauga, the receiving units use checklists specific to their area, so there are many unit based checklists in use. The transfering PACU nurse reviews the PACU record when providing phone report. (Heather Ead)
