Antibiotics Perioperatively

For total joint pts, when the pt has a penicillin allergy,what is your antibiotic of choice? Clindamycin or vancomycin or ?
Secondly, we have had a recent bunch of patients who have become hypothermic once they reach the nursing units.They had a reasonable core temp pre discharge from PACU.It does not seem to be anesthetist specific, thermometers in both areas are calibrated correctly, we are just wondering if anyone else has seen this and if so, what did you implement to "fix it".Appreciate all feedback, stay warm!

Our surgeons would order Vancomycin if a true allergy to penicillin.

We have not had issues with hypothermia on the floor. I would question the technique staff are using, as improper placement of thermometer (not well place in ear canal or under tongue with oral route) can make a difference. If the pt was normothermic in PACU, one wouldn't expect to see other wise on the in-pt units.

What temperature is required for d/c from PACU? I am currently collecting data on firts temperature on arrval to PACU to see if we can meet ASPAN standard of 36C without significant impact on length of stay. Most of our anesthesiologists are pre-warming for major cases and using measures to prevent intraoperative hypothermia, based on Safer Health Care Now recommednations.

It would be interestuing to know what method of temperature measurement was used both in the pACU and the IP unit. The survey that the 2006-2008 ASAPAN standard is based on is dated 1998. Studies included in my recent lit review found that tympanic thermometry is not an 'accurate and reliable estimate' of core temperature.

Is anyone using temporal artey infraed thermometry?
